Callus on ball of foot under big toe

I have a large callus below my big toe on the ball of my foot. I have a custom made shoe insert which was supposed to have alleviated the problem but hasnt, in fact, the callus is worse and pressure there is quite painful. Would a podiatrist be more helpful than an orthopedic surgeon has been, or would it be more likely that the shoe insert was made incorrectly by the orthotist?
