White Spot on Ball of Foot

My 18-yr-old grandson has a spot on the ball of his foot about the size of a half dollar. When his foot is dry, the spot is a little pinker/bluer than the rest of the skin. When his foot gets wet, the spot turns completely white and has a grainy look and feel to it. Do you have any idea what this is? Should we be concerned? Thanks very much.
