WEIRD SWOLLEN ANKEL

My mother had for the past few years on and off swollen ankel it gets worse when she flies but recently it has taken a turn for the worse and is constantly swollen she is in her mid 40 and has taken many test and the docters are baffeled she has taken xray sonograms and they dont see anything would you maybe know what this is
