red swollen toe

My husband's 4rth toe is all red & very swollen.It is very painful & the Dr. he went to the other day says she doesn't know what is wrong with it. She said it wasn't gout or arthritis. Had it x-rayed (not broken). It has been for one & one-half weeks now & no better. Any ideas what it could be. Thank you Marilyn
